Description
Plantar soft tissues of the male foot are presented with the flexor digitorum brevis forming a broad, central muscular belly in the superficial layer, deep to the thick plantar aponeurosis and superficial to the quadratus plantae and long flexor tendons. Distally, the muscle divides into four tendinous slips toward digits II to V, where each tendon typically bifurcates to pass on either side of the flexor digitorum longus tendon before inserting on the sides of the middle phalanx. Medially, the hallux region relates to the flexor hallucis brevis and abductor hallucis along the medial longitudinal arch; laterally, the abductor digiti minimi borders the lateral column. Bony reference points include the calcaneal tuberosity proximally and the metatarsals and phalanges distally. This plantar perspective matters because flexor digitorum brevis sits directly in the line of tension of the plantar fascia and is frequently discussed when teaching the windlass mechanism and support of the longitudinal arches during gait. Pain under the heel and midfoot often gets labeled plantar fasciitis, but clinicians also consider intrinsic muscle strain and myofascial trigger points in flexor digitorum brevis, which can refer pain toward the lesser toes and mimic metatarsalgia. The relationship between the brevis tendons and the flexor digitorum longus is also a clean setup for explaining claw toe mechanics and why imbalance between intrinsic and extrinsic flexors can worsen lesser toe deformity.
