Presented from the plantar (inferior) aspect of a male foot, the flexor digitorum brevis lies just deep to the plantar skin and superficial fascia, occupying the central compartment between the medial abductor hallucis and lateral abductor digiti minimi. Proximally, its broad origin arises from the medial process of the calcaneal tuberosity and the deep surface of the plantar aponeurosis, then courses anteriorly (distally) as a muscular belly over the plantar surface of the cuboid and lateral cuneiform region. Distally, four tendons diverge toward the lateral four digits (II to V), passing plantar to the metatarsophalangeal joints and splitting near the bases of the middle phalanges to allow the flexor digitorum longus tendons to continue to the distal phalanges. Clear tendon anatomy. This plantar view matters because the flexor digitorum brevis is a reliable landmark in layered dissection of the sole and in understanding toe flexion mechanics at the proximal interphalangeal joints, where it counterbalances the long flexors during stance and push-off. Pain at the medial calcaneal tubercle is often labeled plantar fasciitis, but the same region also anchors flexor digitorum brevis and can be involved in enthesopathy or partial tearing after sudden increase in running volume. When exploring chronic heel pain, this relationship helps frame why symptoms may persist despite treatment aimed only at the plantar aponeurosis, and why Baxter nerve (inferior calcaneal nerve) irritation may coexist in the deep interval adjacent to this muscular origin.
