Description
Viewed from the plantar aspect of the male foot with the skin removed, the quadratus plantae (flexor accessorius) occupies the deep central sole just anterior to the calcaneal tuberosity. Its medial head arises from the medial surface of the calcaneus and adjacent plantar fascia, while the lateral head originates from the lateral calcaneus and the long plantar ligament, converging distally to join the tendon of flexor digitorum longus. Superficial to it you would expect the flexor digitorum brevis and plantar aponeurosis, with the calcaneus posteriorly, the metatarsals and phalanges anteriorly, and the neurovascular bundle (medial and lateral plantar nerves and vessels) coursing on either side of the muscle belly. Deep in the sole. Quadratus plantae matters because it corrects the oblique pull of flexor digitorum longus, redirecting force toward the long axes of digits 2 to 5 during late stance and toe-off, which is why its relationships to the calcaneus and to the FDL tendons are a teaching landmark in layer-by-layer plantar dissections. The muscle lies close to the lateral plantar nerve and its first branch (often called Baxter nerve), a frequent pain generator in chronic plantar heel pain that can mimic plantar fasciitis and may be encountered during medial or lateral plantar approaches to the hindfoot. For tendon transfer planning and for explaining clawing patterns after tibial nerve injury, this is the level where the mechanics become obvious.
