Description
Plantar soft tissues are presented from the sole with emphasis on the lateral band of the plantar aponeurosis, the thickened fascial sheet arising from the medial process of the calcaneal tuberosity and coursing anteriorly along the lateral plantar aspect of the foot. From posterior to anterior, the band tracks superficial to the intrinsic musculature and lies inferior to the calcaneus, cuboid, and fifth metatarsal, its distal fibers blending toward the plantar plates and fibrous digital sheaths of the lateral toes. Bony landmarks include the calcaneus and talus proximally and the lateral column tarsals and metatarsals distally, orienting the fascial band along the fifth ray. Clear boundaries. Clinically, this lateral portion of the plantar fascia matters when you are sorting lateral plantar pain from classic medial plantar fasciitis, since traction at the calcaneal origin and strain along the lateral column can mimic peroneal tendon or cuboid pathology. The relationship of the band to the fifth metatarsal region also makes it relevant in cavus foot mechanics and in patients who compensate laterally after an ankle sprain, increasing tensile load through the lateral band during toe-off. Many corticosteroid injections and surgical releases aim to spare excessive fascial disruption, so appreciating where the lateral band runs relative to the calcaneal tuberosity and lateral column helps avoid over-release and iatrogenic arch instability.
