Description
Running along the lateral aspect of the male knee, the fibular (lateral) collateral ligament appears as a taut, cord-like band extending from the lateral femoral epicondyle to the head of the fibula. From an anterior perspective, it lies lateral to the tibial plateau and lateral meniscus region, spanning the joint line as a near-vertical structure. The distal attachment sits on the anterolateral surface of the fibular head, just inferior to the lateral joint line, with the tibia positioned medial to the fibula in the leg. Articular cartilage caps the distal femur and proximal tibia, and the patella occupies the anterior midline in front of the femoral trochlea. This view matters because the lateral collateral ligament is the primary restraint to varus angulation of the knee, and it is a key landmark when you are sorting isolated LCL injury from posterolateral corner injury involving the popliteus tendon, popliteofibular ligament, and biceps femoris insertion. Varus stress testing in near extension and at 30 degrees flexion targets this structure, while abnormal gapping that persists in extension raises concern for combined injury patterns and associated cruciate ligament compromise. Small structure, big diagnostic consequences.
