Description
Running as a continuous band along the anterior surfaces of the vertebral bodies, the anterior longitudinal ligament spans from the basilar part of the occipital bone and anterior tubercle of C1 inferiorly toward the anterior surface of the sacrum, shown here in lateral profile against the cervical, thoracic, and lumbar curvatures. Its fibers broaden and thicken over each intervertebral disc, then narrow as they cross the mid-vertebral body, a pattern that reads clearly from the side when the vertebral bodies and disc spaces are stacked in anatomical alignment. The skull, mandible and dentition, and the upper pelvis with the iliac crest provide orientation for craniocaudal level, while adjacent blue structures suggest paraspinal soft tissues tracking parallel to the column. Clinically, this ligament is the primary anterior restraint to spinal extension and hyperlordosis, so its disc-level thickening matters when teaching segmental motion and why osteophytes form along the anterior vertebral margins in spondylosis. From a lateral perspective you can also connect the anterior longitudinal ligament to cervical anterior approaches, where dissection proceeds medial to the carotid sheath and longus colli to reach the anterior vertebral bodies and disc spaces for discectomy and fusion. This is where anterior bridging osteophytes in diffuse idiopathic skeletal hyperostosis can be explained as ligamentous ossification that preferentially spans disc spaces. Course directors in gross anatomy, kinesiology, and spine biomechanics can pair this view with flexion-extension diagrams to anchor the concept of anterior tension banding across the spine.
