Description
Centered on the anterior hip capsule, the iliofemoral ligament fans from the anterior inferior iliac spine and adjacent acetabular rim toward the intertrochanteric line of the proximal femur, forming its characteristic inverted Y. Its lateral band courses inferolaterally toward the greater trochanteric side of the intertrochanteric line, while the medial band runs more inferomedially toward the lesser trochanteric side, spanning the front of the femoral neck. Surrounding bony landmarks include the ilium, pubis, and ischium framing the acetabulum, with the lumbar vertebral bodies and intervertebral discs superior and posterior to the pelvic ring. Anterior capsuloligamentous anatomy matters because the iliofemoral ligament is the primary passive restraint to hip extension and anterior translation, the reason a standing hip can “lock” with minimal muscular effort. It also explains common exam findings, for example a fixed flexion contracture of the hip that limits extension in osteoarthritis or after prolonged immobilization, and it is directly encountered during anterior approaches to the hip where capsulotomy and repair influence postoperative stability.
