Description
Running along the anterior surfaces of the vertebral bodies from the basilar part of the occipital bone toward the anterior sacrum, the anterior longitudinal ligament is rendered as a dense, continuous band on a lateral view of a male skull and spine. Superiorly it blends with the anterior atlanto-occipital membrane at the craniocervical junction, then descends anterior to the cervical, thoracic, and lumbar vertebral bodies and intervertebral discs. The ligament lies anterior to the vertebral canal and spinal cord, while the spinous processes and posterior elements sit posterior to it. Its fibers appear thickest across the midline of the vertebral bodies and broaden over the discs. Clinically, this is the principal restraint to spinal extension and a key landmark in anterior approaches to the cervical and thoracolumbar spine, where dissection proceeds through prevertebral fascia to the anterior vertebral column. In trauma, hyperextension injury can disrupt the anterior longitudinal ligament and contribute to instability, and in diffuse idiopathic skeletal hyperostosis (DISH) the ligament often ossifies along the anterolateral thoracic spine, producing flowing anterior osteophytes visible on lateral radiographs. Orientation matters. A lateral perspective makes it easier to appreciate how the ligament bridges multiple motion segments while maintaining continuity across cervical lordosis, thoracic kyphosis, and lumbar lordosis.
