Description
Running along the anterior surfaces of the vertebral bodies, the anterior longitudinal ligament (ALL) is presented as a continuous band from the inferior aspect of the occiput and atlas region down to the anterior sacrum. An anterior oblique angle keeps the vertebral column in full length, with the ligament centered on the midline and draped over the intervertebral discs while spanning from one vertebral body to the next. Superiorly the skull anchors the craniovertebral junction in context, and inferiorly the pelvis frames the lumbosacral transition where the ALL broadens over the promontory. Blue highlighting isolates the ligamentous course against the skeletal column. Clinically, the ALL is a key restraint to spinal extension and a frequent landmark in anterior approaches to the cervical and lumbar spine, where dissection proceeds through prevertebral planes to reach disc spaces. Ossification of the anterior longitudinal ligament, including the flowing anterior osteophytes of diffuse idiopathic skeletal hyperostosis (DISH), can be taught cleanly from this viewpoint because it follows the same ventral contour shown here. The same anatomy also underpins hyperextension injury patterns, where disruption of the ALL signals anterior column failure and influences stability assessment.
