Oriented from a medial perspective, the proximal tibia and fibula are presented with the fibular head seated posterolateral to the lateral tibial condyle at the superior tibiofibular joint. Crossing the anterior aspect of that joint, the anterior fibular head (anterior superior tibiofibular) ligament forms a dense oblique band running from the anterolateral tibia to the anterior margin of the fibular head, blending with the local joint capsule and adjacent fascia. Superiorly, the distal femur and its condyles frame the tibiofemoral articulation, while the menisci sit interposed on the tibial plateau. Attention to this ligament matters because the superior tibiofibular joint is not a passive bystander during knee motion, it accommodates torsional forces transmitted from the ankle and helps manage lateral compartment mechanics during flexion and rotation. Injury patterns are most often discussed in the setting of traumatic subluxation or dislocation of the proximal tibiofibular joint, where anterior ligament disruption can allow anterolateral translation of the fibular head and provoke lateral knee pain. Proximity to the common fibular (peroneal) nerve as it wraps around the fibular neck also makes this region clinically unforgiving. Small structure, real consequences.
