Description
Seen from a posterior perspective into the intercondylar region of the knee, the anterior cruciate ligament (ACL) spans from the anterior intercondylar area of the tibia to the medial surface of the lateral femoral condyle. Its fibers course superiorly and posteriorly, twisting as they ascend within the intercondylar notch, with the femoral attachment positioned proximal to the tibial footprint and slightly lateral in relation to the midline of the tibial plateau. Articular cartilage caps the femoral condyles and the tibial plateau, framing the intracapsular ligament against smooth white joint surfaces. No patella. A posterior view is the cleanest way to read the ACL’s obliquity and femoral footprint without the patellofemoral mechanism dominating the field, and it maps directly to what you see during notch visualization in knee arthroscopy. The spiral fiber orientation explains the ligament’s restraint of anterior tibial translation and internal rotation, the motions that fail during a classic pivot-shift injury and drive the decision-making around ACL reconstruction tunnel placement. Small errors here matter. Use this illustration in orthopaedic and sports medicine teaching to differentiate the ACL from the posterior cruciate ligament by attachment site and fiber direction, and in surgical education materials to discuss intercondylar notch anatomy, graft positioning, and common mechanisms of rupture in a male knee.
