Description
Posterior anatomy centers on the vertebral column, with thoracic vertebrae giving rise to ribs that arc laterally and scapulae resting over the upper posterior thorax. Between adjacent transverse processes, the intertransverse ligaments form sparse fibrous bands spanning the intertransverse spaces from cervical through lumbar levels, lying lateral to the laminae and medial to the costal elements in the thorax. Inferiorly, the sacrum sits in the midline between the posterior iliac crests, with the pelvis framing the lumbosacral junction. Intertransverse tissue matters most where it becomes a surgical and procedural landmark. During posterior lumbar approaches such as transforaminal lumbar interbody fusion and during extraforaminal decompression, the intertransverse ligament and adjacent intertransverse membrane are encountered after subperiosteal exposure of the transverse processes; splitting or resecting this band helps access the intertransverse plane while keeping orientation relative to the facet joint medially and the psoas and lumbar plexus laterally. It also frames the corridor used for lumbar transverse process blocks and other paraspinal injections. Small structure, real consequences.
