Description
Shown from the dorsal (posterior) aspect of an adult male hand, the interosseous ligamentous and fascial planes are emphasized against the carpal, metacarpal, and phalangeal skeleton. Extensor tendons course distally over the dorsum of the metacarpals toward the dorsal digital expansions, while the highlighted bands correspond to capsuloligamentous structures spanning adjacent bones along the intermetacarpal and carpometacarpal intervals. Proximally, the carpal bones form the base of the construct; distally, the metacarpophalangeal and interphalangeal joints align in series, with ligamentous thickening positioned deep to the extensor apparatus and superficial to the dorsal aspects of the bones. Orientation stays strictly posterior, with radial structures lateral and ulnar structures medial in anatomical position. A posterior hand view is where dorsal stability and tendon gliding are easiest to teach, because you can relate the interosseous bands and dorsal joint capsules directly to the extensor mechanism that crosses them. This is the side surgeons use when planning and explaining dorsal approaches to the metacarpals and phalanges (for fracture fixation, dorsal capsulotomy, or extensor tendon repair), where excessive dissection can stiffen the metacarpophalangeal joint by scarring the capsule and dorsal interosseous fascia. Dorsal ligamentous injury also matters in high-energy carpometacarpal trauma and in collateral ligament disruptions that destabilize the extensor hood. Small structures, big consequences.
