Posteriorly, the right ankle is opened to emphasize the posterior tibiotalar part of the medial (deltoid) ligament, highlighted against the distal tibia, fibula, and the trochlea of the talus. Fibers arise from the posterior colliculus of the medial malleolus and sweep inferolaterally and distally in a fan-like band to the posteromedial talus, spanning the tibiotalar joint line. The ligament sits medial to the fibula and posterior to the talar dome, with adjacent capsular tissue and periosteum blending at its margins. Bone landmarks stay crisp.
