Description
Arising from the medial malleolus of the distal tibia, the tibiocalcaneal part of the medial (deltoid) ligament descends inferiorly and slightly posteriorly to insert on the medial calcaneus at the sustentaculum tali. Deep to this band lies the talus, with the calcaneus positioned inferior to the talar body and neck, forming the medial wall of the ankle and subtalar region. The medial view clarifies how the ligament sits medial to the ankle mortise, bridging tibia to calcaneus while running anterior to the Achilles tendon insertion on the posterior calcaneal tuberosity. Fibers appear as a broad, thickened sheet of fascia-like collagen rather than a cord. This perspective matters when teaching or planning around medial ankle stability, because the tibiocalcaneal band is one of the primary restraints to valgus tilt of the talus and excessive eversion at the subtalar joint. Injury patterns differ from the lateral ligament complex: deltoid disruption often accompanies ankle fractures or external rotation mechanisms, and tenderness along the medial malleolus to sustentaculum corridor raises concern for medial instability even when radiographs look subtle.
