Description

Oriented in lateral anatomical position, the distal tibia and distal fibula form the mortise around the trochlea of the talus, with the posterior tibiofibular ligament spanning between the posterior margin of the tibial incisura and the posterior aspect of the lateral malleolus. From this side-on perspective, the ligament sits posterior to the distal tibiofibular syndesmosis and deep to the superficial contours of the lateral malleolus, while the talus and calcaneus define the hindfoot immediately inferior to the joint line. More anteriorly in the lateral complex, the anterior talofibular and calcaneofibular ligaments are logically positioned relative to the fibula, and the Achilles tendon inserts on the posterior calcaneal tuberosity. The plantar fascia tracks along the plantar aspect of the foot, inferior to the calcaneus and longitudinal arch. Posterior syndesmotic anatomy matters because many ankle sprains that look like routine lateral ligament injuries actually involve the distal tibiofibular ligaments, and the posterior tibiofibular ligament is a principal restraint to posterior translation and external rotation of the fibula relative to the tibia. This is the structure stressed in high ankle sprain mechanisms and the one surgeons inspect when deciding between trans-syndesmotic screws versus suture-button fixation, since unrecognized posterior instability can leave the talus subtly malreduced in the mortise.
