Description
Posterior to the distal tibia and fibula, the posterior tibiofibular ligament spans obliquely from the posterior tibial malleolar region to the posterior aspect of the lateral malleolus, reinforcing the distal tibiofibular syndesmosis. The inferior transverse tibiofibular component forms a stout ridge at the posterior margin of the tibial plafond, lying immediately superior to the trochlear surface of the talus. Inferiorly, the talus sits under the tibial plafond with the calcaneus posterior and plantar, and the proximal tarsus leads into the metatarsus distally. A blue-highlighted tendon or muscle belly tracks along the posterior ankle near the retromalleolar groove, placed to emphasize its relationship to the syndesmotic ligament complex. Tight anatomy. Syndesmotic integrity matters most when you are sorting out a high ankle sprain from a routine lateral ligament injury, and the posterior tibiofibular ligament is the main checkrein against posterior and external rotation displacement of the fibula relative to the tibia. On MRI and arthroscopy, the inferior transverse portion can mimic a loose body or create a posterior recess that traps synovium, and in posterior malleolar fractures its attachment helps explain why a small fragment can destabilize the ankle mortise. This posterior perspective also clarifies the relationship of the ligament to the posterior talar dome and the retromalleolar tendons, a common source of diagnostic confusion on axial imaging.
