Description
Plantar exposure of the midfoot centers on the cuboideonavicular ligament spanning from the plantar aspect of the cuboid to the plantar surface of the navicular, bridging the lateral and medial columns just anterior to the calcaneus. The cuboid sits lateral and distal to the talus and calcaneus, while the navicular lies medial and distal to the talar head, proximal to the three cuneiforms. Distal to the ligament, the bases of the metatarsals align with the tarsometatarsal joints, and the phalanges extend anteriorly. Small but telling. This relationship frames the transverse tarsal (midtarsal) region in true plantar orientation. Clinically, the cuboideonavicular band matters when you need to explain midfoot stability beyond the better known plantar calcaneonavicular (spring) ligament and the long plantar ligament. Disruption across the midfoot, as in Lisfranc complex injuries or crush mechanisms, can propagate into the lateral column and alter load transfer through the calcaneocuboid and talonavicular joints, with pain often localized to the plantar midfoot. For surgeons and radiologists, the plantar perspective helps correlate the deep ligamentous restraints with plantar ecchymosis and with MRI or ultrasound planes that track along the plantar ligaments rather than the dorsal capsules. Use this asset for teaching midfoot ligament anatomy in gross anatomy, podiatry, or orthopaedic foot and ankle modules, where students must differentiate plantar ligament layers and relate them to the medial and lateral longitudinal arches. It also fits well in operative atlases and patient education pieces discussing midfoot sprain patterns, arch support biomechanics, or post traumatic midfoot instability.
