Description
Plantar anatomy of the male forefoot is rendered with emphasis on the deep transverse metatarsal (intermetatarsal) ligament spanning the distal metatarsal heads. The ligament bridges adjacent metatarsals from medial to lateral, lying plantar to the metatarsophalangeal joints and deep to the flexor tendon plane, where it stabilizes the forefoot during toe-off. Proximally, the metatarsal shafts extend toward the tarsus, with the ankle region implied at the posterior margin as the bony architecture transitions into the midfoot. That transverse restraint matters because it defines the intermetatarsal spaces and the mechanics of splay, and it is a key landmark in Morton neuroma, where the common plantar digital nerve most often becomes symptomatic in the third interspace just plantar to this ligament. Surgeons often release the deep transverse metatarsal ligament during decompression or neurectomy, and the relationship to the metatarsal heads helps explain why pain localizes between toes and worsens with narrow footwear.
