Seen from the plantar aspect of the male foot, the plantar calcaneonavicular ligament (spring ligament) spans from the anterior margin of the calcaneus at the sustentaculum tali to the plantar surface of the navicular, forming a fibrocartilaginous sling directly inferior to the head of the talus. The talus sits superior and slightly posterior to the navicular, while the calcaneus lies posterior and lateral, creating the medial column of the tarsus. Distal to this complex, the cuneiforms align with the bases of the metatarsals, situating the spring ligament proximal to the tarsometatarsal region and medial to the lateral plantar ligaments. Fine connective tissue bands may be shown blending with the joint capsule of the talocalcaneonavicular articulation. Short and dense. Functionally, this plantar ligament is the primary passive buttress against plantar and medial sag of the talar head, so this inferior view is the clearest way to understand how it supports the medial longitudinal arch during stance and push-off. Failure or attenuation of the spring ligament complex is a core structural lesion in adult acquired flatfoot deformity (progressive collapsing foot deformity), often accompanying posterior tibial tendon dysfunction, with resultant talonavicular uncovering and hindfoot valgus. Surgeons repair or reconstruct it in conjunction with procedures such as medializing calcaneal osteotomy or tendon transfer, and radiologists correlate it with MRI findings of thickening, tearing, or periligamentous edema at the sustentaculum tali and navicular insertions.
