Plantar anatomy of the right forefoot centers on the superficial transverse metatarsal ligament spanning the plantar plates at the metatarsophalangeal (MTP) joints, bridging adjacent metatarsal heads across the distal metatarsus. The band runs transversely, deep to the digital fibrous sheaths and flexor tendons of flexor digitorum longus and flexor digitorum brevis, and superficial to the deep transverse intermetatarsal ligaments and interossei. Medial and lateral to this plane, the first and fifth MTP regions frame the transverse arch, while the proximal phalanges lie distal to the ligament as it stabilizes the toe rays.
