Description
Plantar anatomy of the male foot is rendered with the calcaneus positioned posteriorly and the navicular immediately anterior and medial, bridged by the plantar calcaneonavicular (spring) ligament along the inferomedial margin of the talonavicular region. The talar head sits superior to this fibrocartilaginous band, while the cuboid and lateral column lie more laterally and the metatarsals extend distally toward the phalanges. Adjacent stabilizers such as the long plantar ligament and portions of the plantar fascia are logically appreciable in the same plane, with tendinous structures coursing longitudinally toward the forefoot. Orientation is clear. Clinically, the spring ligament forms the floor for the talar head and works with the posterior tibial tendon to resist collapse of the medial longitudinal arch, so this perspective is the one you want when teaching acquired flatfoot deformity (progressive collapsing foot deformity) and the mechanics of talonavicular uncovering. Attenuation or rupture at the calcaneal or navicular attachments allows the talar head to plantarflex and adduct, often accompanying posterior tibial tendon dysfunction and producing hindfoot valgus and forefoot abduction.
