Description
Seen from the lateral aspect of the male foot and ankle, the talus sits superior to the calcaneus and articulates anteriorly with the navicular, while the calcaneus extends posteriorly to form the heel and anteriorly toward the calcaneocuboid joint. Deep within the sinus tarsi between the talar sulcus and calcaneal sulcus, the interosseous talocalcaneal ligament spans obliquely, binding the subtalar joint surfaces and separating the posterior subtalar facet from the anterior and middle facets. Portions of the lateral malleolus (fibula) and distal tibia are typically visible superiorly, with the tarsus leading into the metatarsus and phalanges distally. Bony alignment is clear. This lateral perspective matters because it clarifies how the interosseous talocalcaneal ligament functions as the primary internal stabilizer of the subtalar joint, limiting excessive inversion and eversion while maintaining congruity during gait. Sinus tarsi syndrome and lateral hindfoot pain often follow inversion sprains, and clinicians frequently correlate point tenderness over the sinus tarsi with injury to the interosseous and cervical talocalcaneal ligaments.
