Description
Running along the tips of the lumbar spinous processes, the supraspinous ligament forms a continuous midline band from the upper lumbar spine toward the sacrum, blending inferiorly with the posterior sacral soft tissues near the sacral crest and the coccygeal region. Spinous processes sit deep to the ligament, while the posterior elements of the vertebral arches broaden laterally toward the transverse processes, framing the midline. Inferiorly, the sacrum lies between the right and left ilia, with the sacroiliac joints positioned posterolaterally to the ligament’s course. Proximal femora appear lateral and inferior to the pelvic ring at the hip joints. A posterior perspective on the supraspinous ligament matters when you need to teach or plan around the posterior ligamentous complex (PLC), the structure set most scrutinized in thoracolumbar trauma classification for suspected flexion distraction injury.
