Description
Posterior anatomy centers on the midline spinous processes from the upper cervical spine through the upper thoracic vertebrae, with the interspinous ligament rendered as vertically oriented fibers spanning the interspinous spaces between adjacent processes. Superiorly, the occipital bone frames the craniovertebral junction, and the sagittal and lambdoid sutures remain visible as cranial landmarks above C1 and C2. Lateral to the spinous processes, the ribs begin to arc anterolaterally from the thoracic vertebrae, giving a partial outline of the posterior ribcage. Midline continuity is clear. Emphasis on the interspinous ligament matters because it sits directly between the spinous processes, blending with the supraspinous ligament posteriorly and transitioning toward the ligamentum flavum more anteriorly, so it helps define the posterior ligamentous complex targeted in trauma assessment. Hyperflexion injuries and interspinous edema on MRI often track along these fibers, and widening of an interspinous space can flag posterior instability even when the vertebral bodies appear aligned. Pain generators also live here, since interspinous bursitis and degenerative strain can localize tenderness between spinous processes in the cervical or upper thoracic region.
