Description
Seen from the anterior aspect of a male knee, the distal femur sits superior to the tibial plateau, with the patella positioned anteriorly over the trochlear groove. Lateral to the tibia, the head of the fibula lies inferior and slightly posterior to the lateral tibial condyle, and a stout fibrous band, the anterior fibular head (anterior superior tibiofibular) ligament, spans between the anterolateral tibia and the anterior margin of the fibular head. Adjacent stabilizers you would expect in this field include the fibular (lateral) collateral ligament running from the lateral femoral epicondyle to the fibular head and the iliotibial tract coursing distally toward Gerdy’s tubercle. A clear anterior landmark. This anterior perspective matters because the proximal tibiofibular joint is a frequent blind spot in knee assessment, yet it can be a pain generator and a site of instability after twisting injuries or lateral knee trauma. The anterior fibular head ligament resists anterolateral translation of the fibular head and helps constrain coupled motion between the fibula and tibia that affects peroneal nerve mechanics, posterolateral corner tensioning, and even the kinematics of high tibial osteotomy planning. Tenderness or prominence at the fibular head also raises the differential of proximal tibiofibular joint sprain, subluxation, or iatrogenic injury near the common fibular (peroneal) nerve as it wraps around the fibular neck.
