Centered in the distal palm, the distal commissural ligament (distal transverse metacarpal ligament) spans transversely between the fibrous digital sheaths at the level of the metacarpophalangeal region, tethering the adjacent flexor tendon canals of the index through little fingers. Distal to the metacarpal heads and proximal to the proximal phalanges, it sits superficial to the deep transverse metacarpal ligament and helps maintain the interdigital spacing on the volar aspect. In an anterior orientation, you would expect the flexor digitorum superficialis and profundus tendons within their synovial sheaths, the A1 pulley region, and the palmar aponeurosis to frame the ligament, with metacarpals and proximal phalanges providing the osseous backdrop. Small neurovascular bundles course along the radial and ulnar sides of each digit. This is a high-yield structure when teaching why the fingers move as a coordinated unit rather than as four independent rays. The distal commissural ligament resists excessive splaying at the metacarpophalangeal joints and becomes clinically relevant in Dupuytren disease, where pretendinous and commissural cords can contract and narrow the first web space or tether the interdigital commissures. It also matters during A1 pulley release for trigger finger, because dissection that strays too far laterally risks the digital nerves and arteries that run close to the fibrous sheath and adjacent commissural attachments.
