Description
Oriented from the lateral aspect of the knee, the fibular head sits distal and lateral to the lateral tibial condyle, forming the superior tibiofibular joint at the posterolateral corner of the proximal leg. The anterior fibular head ligament is highlighted as a fibrous band spanning from the anterolateral tibia to the anterior margin of the fibular head, running slightly oblique across the joint capsule. Nearby landmarks that logically frame this structure include the fibular collateral ligament descending from the lateral femoral epicondyle toward the fibular head and the biceps femoris tendon inserting on the fibular head just posterior to the anterior ligament. Articular margins and periosteal attachments are kept crisp. Small but specific. Stability of the superior tibiofibular joint is easy to ignore until it fails, and this lateral view is one of the better ways to teach the anterior restraint to anterior translation of the fibular head during ankle and knee motion. Injuries after an ankle inversion mechanism, high-energy twisting, or tibial plateau trauma can disrupt this ligament and contribute to painful proximal tibiofibular instability, often with peroneal nerve symptoms given the nerve’s course around the fibular neck. Surgeons also care about the relationship between the ligament, the biceps femoris insertion, and the fibular collateral ligament when planning posterolateral corner reconstruction or exposure around the fibular head.
