Description
Emerging across the posterior scapula, the infraspinatus occupies the infraspinous fossa inferior to the scapular spine, its fibers converging laterally toward the greater tubercle of the humerus as part of the rotator cuff. Superficial to it lie the posterior deltoid laterally and the trapezius and posterior shoulder fascia medially, while teres minor runs along its inferior lateral border and teres major sits more inferiorly near the posterior axillary fold. Midline landmarks of the back, including the spinous processes of the cervical and thoracic vertebrae, help orient the scapulae and the posterior shoulder girdle in anatomical position. Bony contours remain readable beneath the semi-transparent soft tissues. Clinically, this posterior relationship is the one you need when teaching external rotation strength testing and when explaining why isolated infraspinatus atrophy points to suprascapular nerve compromise at the spinoglenoid notch rather than the suprascapular notch.
