Beneath an intact posterior skin envelope, the infraspinatus occupies the infraspinous fossa of the scapula, inferior to the scapular spine and lateral to the medial border, with fibers converging superolaterally toward the greater tubercle of the humerus as part of the rotator cuff. Its lateral margin approaches the posterior deltoid, while inferiorly it borders teres minor and transitions toward the teres major and latissimus dorsi contour over the posterior axillary fold. Bilateral surface musculature remains visible in gray across the back, pelvis, and thighs, while the shoulder region is selectively emphasized in red to separate posterior deltoid fibers from the underlying scapular stabilizers. External rotation lives here. Loss of infraspinatus bulk or altered fiber direction is a common teaching sign for rotator cuff pathology, and on the posterior shoulder it helps you localize deficits that distinguish infraspinatus weakness from isolated teres minor involvement. The posterior perspective also supports discussion of suprascapular nerve entrapment at the spinoglenoid notch, a pattern that can present with selective infraspinatus atrophy and posterior shoulder pain in overhead athletes. Palpation, dry needling targets, and posterior glenohumeral injection planning all benefit from seeing how the muscle lies deep to the deltoid but superficial to the scapular body.
