Rendered from a posterior perspective, the proximal humerus is presented in situ with the scapula and posterior thoracic cage as landmarks. The humeral head and anatomical neck sit superomedial to the surgical neck region, while the greater tubercle lies more lateral and the lesser tubercle remains anterior and largely out of direct view. A fracture line is marked near the proximal metaphysis, extending toward the anatomical neck, with the humeral shaft continuing distally in the brachium. Posterior orientation matters for proximal humerus trauma because it clarifies the relationship of the fracture to the glenohumeral joint surface and the tuberosities that anchor the rotator cuff (supraspinatus, infraspinatus, teres minor, subscapularis). Displacement patterns often track muscle pull, and a fracture approaching the anatomical neck raises concern for disruption of the arcuate artery branch of the anterior circumflex humeral artery, a mechanism implicated in humeral head osteonecrosis after anatomic neck fractures. For operative planning, this view also helps explain why axillary nerve injury is classically discussed with proximal humeral fractures, given its course around the surgical neck and proximity to posterior approaches.
