Posterior aspects of the male humerus are presented with a clean transverse fracture through the mid-diaphysis. Proximally, the humeral head and surgical neck lie superior to the shaft, while distally the medial and lateral epicondyles bracket the olecranon fossa and the posterior margin of the trochlea. The fracture line crosses the shaft near the radial (spiral) groove, which runs obliquely inferolaterally on the posterior surface and sits lateral to the long axis of the bone. Two fragments are separated at the break. A transverse midshaft humeral fracture carries a specific neurovascular implication because the radial nerve and profunda brachii artery track in the spiral groove and can be tethered at the fracture site or compressed by callus during healing. Posterior orientation makes the relationship of the fracture to the groove easier to teach than an anterior view, and it matches the mental map used when assessing wrist drop and sensory loss over the dorsoradial hand after arm trauma.
