Description
Viewed from the posterior (dorsal) aspect of the hand, the dorsal interosseous muscles occupy the intermetacarpal spaces as bipennate muscle bellies arising from adjacent metacarpal shafts. Their tendinous insertions pass distally toward the bases of the proximal phalanges and blend with the dorsal digital expansion (extensor hood), lying deep to the extensor digitorum tendons as those tendons course centrally over the metacarpals. Ulnar and radial-sided interossei can be inferred by their position relative to the third metacarpal, the functional axis for finger abduction. Deep plane anatomy. This posterior perspective matters because it clarifies how the interossei act as ABductors at the metacarpophalangeal joints while also contributing to interphalangeal extension through the extensor mechanism, a relationship that becomes clinically apparent in ulnar nerve palsy when interosseous denervation produces dorsal guttering and clawing (hyperextension at the MCP joints with IP flexion).
