Positioned on the posterolateral scalp, the auricular musculature lies superficial to the mastoid part of the temporal bone and posterior to the cartilaginous auricle (pinna). Prominent slips would include the auricularis posterior arising from the mastoid region and coursing anterolaterally toward the posterior auricular surface, with the auricularis superior spanning superiorly toward the epicranial aponeurosis (galea aponeurotica) and the auricularis anterior approaching the root of the helix from the temporal fascia. Small intrinsic muscles, including helicis major and minor, tragicus, antitragicus, transverse auricular, and oblique auricular muscles, would be expected to contour the cartilage on its lateral surface, while the posterior auricular artery and posterior auricular nerve travel just posterior to the auricle. Landmarks matter here. The external acoustic meatus sits anterior to the mastoid process, deep to the auricular attachments. A posterior perspective clarifies the relationship of these thin, variably developed muscles to the retroauricular skin flap used in otologic surgery, where the incision and elevation plane must respect the posterior auricular neurovascular bundle. It also supports teaching around auricular movement and its limited clinical expression in most adults, while still explaining why focal denervation (facial nerve involvement near the stylomastoid foramen) can alter peri-auricular tone and the patient’s ability to “wiggle” the ear. This is a common point of confusion when learners conflate extrinsic auricular muscles with the middle and inner ear. Different compartments, different functions.
