Description
Posteriorly, the lumbar vertebrae (L1 to L5) descend to the sacrum and coccyx, framed laterally by the iliac crests and posterior iliac spines. Draped across the dorsal elements, the external vertebral venous plexus forms a longitudinal vascular network over the laminae and spinous processes, communicating segmentally through the intervertebral foramina with veins coursing alongside the spinal nerves. Superiorly, the lowest ribs mark the thoracolumbar junction, while intervertebral discs separate the vertebral bodies anterior to the posterior arch. Because this is a valveless system, the external venous plexus provides a low-pressure conduit between pelvic veins and the azygos system, and it can become a route for hematogenous spread to the vertebral column. Surgeons operating through a posterior midline approach for lumbar decompression or instrumented fusion regularly encounter brisk bleeding from these veins near the facet joints, interlaminar spaces, and foraminal margins. Bleeding control matters. Common teaching points include the relationship of foraminal veins to the exiting dorsal root ganglion, and why increased intra-abdominal pressure can distend the vertebral venous plexus and worsen epidural venous engorgement or contribute to operative blood loss.
