Description
Running longitudinally along the dorsal surfaces of the vertebral arches and spinous processes, the external vertebral venous plexus forms a paired, anastomosing network that parallels the cervical, thoracic, and lumbar spine. Segmental veins connect the plexus to the posterior intercostal and lumbar veins, crossing laterally over the laminae near the intervertebral foramina where spinal nerves emerge. Superiorly, tributaries communicate with the occipital and deep cervical venous systems at the craniovertebral junction, while inferior connections descend toward the sacral venous plexuses over the posterior pelvis. Orientation is unapologetically posterior. Venous anatomy around the vertebrae matters because this plexiform system is valveless and pressure responsive, providing a low resistance collateral pathway between the pelvis, thorax, and skull. That feature underlies classic spread patterns of infection and metastasis, including hematogenous dissemination to the vertebral bodies and epidural space from pelvic malignancies via the vertebral venous system (Batson plexus). For spine surgeons, the posterior approach brings you directly onto these veins at the lamina and facet joints, where brisk, diffuse bleeding can obscure decompression during laminectomy or posterior instrumentation.
