Description
Posterior arm fascia forms a dense sleeve around the triceps brachii, investing the long head medially, the lateral head laterally, and the medial head deep and distal as it converges toward the olecranon. Proximally, the fascial layer blends with the deltoid fascia over the posterolateral shoulder and anchors along the humerus via the lateral and medial intermuscular septa. Inferiorly, it continues into the antebrachial fascia across the elbow, where the triceps tendon passes superficially to insert on the ulna. A firm envelope. Clinically, this posterior compartment fascia matters because it defines the tissue planes you dissect in a posterior approach to the humerus and it helps explain how bleeding or infection can track along the arm into the forearm. Radial nerve injury and profunda brachii artery compromise are classic concerns along the spiral groove deep to the lateral head of triceps, and their relationship to the compartment boundaries guides safe retraction during open reduction and internal fixation of humeral shaft fractures. The same fascial constraints also frame posterior compartment swelling after trauma, where pain with passive elbow motion and tense posterior arm can raise concern for compartment syndrome.
