Description
Arising from the posterior tubercles of the transverse processes of C1 to C4, the levator scapulae descends obliquely and laterally to insert along the medial border of the scapula at the superior angle, just superior to the root of the scapular spine. From a posterior perspective, its muscle belly sits deep to the upper trapezius and lateral to the cervical spinous processes, forming a distinct strap as it crosses the posterolateral neck. Inferiorly, its insertion lies superior to rhomboid minor and adjacent to the superomedial corner of the scapula, a key landmark for orienting shoulder girdle musculature against the cervical spine. Palpation and dry needling often target this muscle because levator scapulae trigger points commonly refer pain to the posterolateral neck and superior medial scapular border, a pattern frequently mistaken for cervical facet pain. Surgical and rehab planning also benefits from this view: the dorsal scapular nerve (C5) typically courses deep to levator scapulae before reaching the rhomboids, and traction or entrapment in this region can produce medial scapular border discomfort and scapular dyskinesis. Its line of pull makes the relationship between ipsilateral scapular elevation and cervical ipsilateral flexion immediately legible. A common pain generator.
