Description
Centered in the posterior knee, the popliteus is presented as a thin, obliquely oriented muscle deep to the superficial fascia and deep to the gastrocnemius heads, forming part of the floor of the popliteal fossa. Its tendon courses superolaterally toward the lateral femoral condyle and knee capsule, while the muscle belly runs inferomedially across the posterior tibia, just superior to the soleal line. Proximally, the biceps femoris lies lateral and the semimembranosus and semitendinosus lie medial, framing the fossa above the posterior joint line. Distally, the gastrocnemius continues into the calcaneal (Achilles) tendon, with the tibia and fibula providing bony reference along the leg. Posterior exposure matters here because the popliteus sits at the crossroads of posterior capsular structures and the posterolateral corner, where injury patterns can be subtle and often missed on surface inspection. Clinically, the popliteus tendon and popliteofibular ligament complex are evaluated in suspected posterolateral rotatory instability, and this view helps clarify why a varus stress injury can spare the cruciates yet still produce functional giving way. It is also a reliable landmark in arthroscopic and open procedures around the lateral compartment, where the tendon’s relationship to the lateral meniscus and capsule guides safe dissection and repair. Small muscle, big consequences.
