Description
Posterior to the elbow joint, the lateral epicondyle of the humerus anchors the radial collateral ligament, its fibers blending laterally with the annular ligament that encircles the head of the radius. The radial head sits inferior to the capitulum, while the olecranon of the ulna occupies the posterior midline, forming the dorsal contour of the joint. Along the posterolateral aspect, the ligamentous band runs obliquely distal toward the proximal radioulnar region, positioned superficial to the joint capsule and adjacent to common extensor tendon attachments. Orientation is clear. Posterior visualization of the radial collateral ligament matters because posterolateral rotatory instability often follows failure of the lateral collateral ligament complex, classically after elbow dislocation or iatrogenic injury during lateral approaches. Clinically, varus stress and a positive posterolateral rotatory instability test implicate the RCL and its continuity with the annular ligament, a relationship that explains why radial head subluxation and symptomatic clicking can accompany lateral elbow pain. For surgical planning, this view supports discussion of ligament repair or reconstruction at the lateral epicondyle and helps differentiate lateral ligament pathology from isolated extensor tendinopathy.
