Description
Posterior cervical skin and superficial fascia are rendered semi-translucent to reveal the semispinalis capitis as a paired, longitudinal muscle mass flanking the midline from the upper thoracic and lower cervical region toward the occiput. Medially, its fibers course adjacent to the cervical spinous processes and nuchal ligament, while laterally it sits deep to the splenius capitis and medial to the more laterally sweeping fibers of trapezius when those layers are implied. Superiorly, the muscle belly thickens as it approaches its attachment between the superior and inferior nuchal lines of the occipital bone. Orientation is a posterior three-quarter view. Semispinalis capitis is the power extensor of the head and a key contributor to contralateral rotation, so its depth relationship to splenius capitis and trapezius matters when teaching layered dissection and palpation accuracy. Clinically, this muscle is a common generator of cervicogenic headache and occipital pain patterns, and it lies in the neighborhood of the greater occipital nerve as that nerve emerges near the superior nuchal line and can be irritated by myofascial tension. This is the muscle students confuse. The posterior perspective also supports discussion of posterior cervical approaches where midline and paramedian muscle splitting, subperiosteal elevation, and retraction can affect postoperative neck pain.
