Description
Arising from the transverse processes of the upper thoracic and lower cervical vertebrae, the semispinalis capitis is highlighted bilaterally as a paired transversospinalis muscle ascending superomedially to insert on the occipital bone between the superior and inferior nuchal lines. From this posterior angle, its fascicles sit deep to trapezius and splenius capitis, forming a muscular column just lateral to the midline over the cervical spinous processes. Inferiorly, the semispinalis cervicis appears as the caudal continuation, positioned medial to the longissimus portion of the erector spinae group. Clear fiber direction. Clinically, this view clarifies why deep posterior cervical pain can persist even when the superficial trapezius feels tender, because semispinalis capitis often becomes a nociceptive generator in cervicogenic headache and whiplash-associated disorders. The depiction also supports teaching of segmental extension and contralateral rotation of the head, actions that become easier to predict when you can trace the muscle from its transverse process origins to its occipital insertion.
