Description
Posterior to the male pelvis and thigh, the biceps femoris occupies the lateral hamstring compartment, with the long head descending from the ischial tuberosity and the short head arising more distally from the linea aspera and lateral supracondylar line of the femur. The short head sits deep and slightly anterior to the long head in the distal half of the posterior thigh, then converges laterally toward the fibular head. Medially, semitendinosus and semimembranosus run in parallel toward the medial tibia, while the femur remains central as the bony axis between hip and knee. Lateral to the knee, the biceps femoris tendon frames the superolateral margin of the popliteal fossa. For clinical teaching, separating the short head from the long head matters because their innervation and injury patterns diverge: the short head receives fibers from the common fibular (peroneal) division of the sciatic nerve, whereas the long head is tibial division. This distinction helps explain focal weakness in knee flexion and external rotation when a common fibular neuropathy accompanies trauma near the fibular neck, and it clarifies why posterolateral knee pain can reflect tendinopathy at the fibular insertion rather than a proximal hamstring strain at the ischial tuberosity. A frequent landmark. The tendon’s relationship to the fibular head also overlaps the surgical corridor used in posterolateral corner reconstruction and in exposure during proximal fibular procedures, where the nearby common fibular nerve is at risk.
