Description
Running along the posterolateral thigh, the long head of the biceps femoris is highlighted from its proximal attachment on the ischial tuberosity of the pelvis to its distal tendon at the head of the fibula, forming the lateral border of the popliteal fossa. Superiorly, the muscle belly lies inferior to the gluteal region and lateral to semitendinosus and semimembranosus, then narrows as it approaches the posterior aspect of the knee. Near the knee joint, its tendon passes posterior to the lateral femoral condyle and toward the fibular head, just lateral to the posterior course of the tibia. Clear surface anatomy. This posterior perspective matters because the long head is the hamstring component most often palpated as the lateral hamstring tendon, a key landmark when teaching knee flexion biomechanics and posterolateral knee anatomy. Strain injuries commonly occur at the proximal myotendinous junction during sprinting, while proximal avulsion from the ischium is a distinct entity in explosive hip flexion with knee extension and can be misread as nonspecific buttock pain. Distally, the tendon’s relationship to the fibular head places it adjacent to the common fibular (peroneal) nerve as it winds around the neck of the fibula, a practical correlation for lateral knee trauma, fibular head fractures, and surgical exposure of the posterolateral corner.
