Description
Posterior lower leg anatomy is presented with the distal femur superiorly, transitioning into the tibia medially and fibula laterally, framing the deep posterior compartment. The soleus occupies a broad, flattened field deep to the gastrocnemius, spanning from the posterior proximal tibia along the soleal line and the proximal fibula, then narrowing distally toward the calcaneal (Achilles) tendon. Medial and lateral borders follow the contours of the tibia and fibula, while the muscle belly sits posterior to both bones and inferior to the femoral condyles. Broad origin, converging distal insertion. Clinically, this posterior perspective supports teaching of the triceps surae as a functional unit, but it also clarifies what differs: the soleus is a single-joint plantarflexor that does not cross the knee, unlike the gastrocnemius arising from the femur. That distinction matters in Achilles tendinopathy and surgical planning, because loading protocols and operative exposure often consider whether symptoms arise with the knee flexed (biasing soleus) versus extended (biasing gastrocnemius). The view also helps orient the interval used in posterior approaches when identifying deeper structures (tibial nerve and posterior tibial vessels) that lie deep and medial to the soleus in the leg.
