Description
Running as a narrow, vertical column immediately adjacent to the thoracic spinous processes, the spinalis thoracis is highlighted as the most medial component of the erector spinae mass on a posterior male trunk. Its fibers lie medial to longissimus thoracis and deep to the superficial contour created by trapezius superiorly, while latissimus dorsi forms a lateral wall over the mid to lower back. Inferiorly, the paraspinal musculature blends into the thoracolumbar fascia above the iliac crests, with gluteus maximus occupying the posterior pelvis and defining the lumbosacral transition. Medial paraspinal anatomy matters when you are teaching segmental extension mechanics and when you are correlating back pain with facet-mediated nociception, because the spinalis and adjacent multifidus occupy the same corridor used for medial branch blocks and radiofrequency ablation planning. Tenderness or denervation-related atrophy in this region is often discussed in chronic low back pain and post-laminectomy syndromes, and the thoracic portion helps explain why scapulothoracic posture and thoracic kyphosis can alter extensor loading patterns.
