Posterior Perspective of the Ulnar Nerve

Description

Posterior to the medial epicondyle of the humerus, the ulnar nerve tracks in the ulnar groove (sulcus nervi ulnaris) before passing between the heads of flexor carpi ulnaris into the posterior compartment of the forearm. Proximally, its course can be followed from the brachial plexus region through the arm, where it runs medial to the humeral shaft and approaches the elbow between the medial intermuscular septum and the triceps brachii. Radius and ulna frame the proximal forearm distal to the elbow joint, with carpal bones, metacarpals, and phalanges extending distally to the ulnar-sided digits. Bony landmarks dominate. The neurilemma and nerve fasciculi are implied by the stylized nerve rendering. Posterior viewing matters at the elbow because this is where the ulnar nerve becomes superficial and vulnerable, a key teaching point for cubital tunnel syndrome and for the familiar paresthesia after striking the “funny bone.” The relationship to the medial epicondyle also guides operative planning for ulnar nerve decompression or anterior transposition, and helps explain why valgus stress, osteophytes, or a thickened Osborne’s ligament can provoke symptoms with elbow flexion. Sensory findings in the small finger and ulnar half of the ring finger, plus weakness in intrinsic hand muscles, make more sense once you see the nerve’s tethered path behind the epicondyle.
