Description

Posterior to the distal humerus, the fibrous joint capsule spans the margins of the olecranon fossa and blends with periosteum along the medial and lateral epicondyles, forming a taut roof over the posterior recess of the elbow. Inferiorly it attaches to the proximal ulna around the trochlear notch and to the annular ligament at the radial head, enclosing the humeroulnar and humeroradial articulations. Laterally, the capsule thickens in continuity with the radial collateral ligament complex near the lateral epicondyle, while medially it reinforces toward the posterior band of the ulnar collateral ligament. Bone landmarks anchor the composition. A posterior capsule view matters because this is the part that gets pinched and inflamed in terminal extension, the typical mechanism behind posterior elbow impingement and osteophyte-related extension pain in throwers and manual laborers. Capsular tension across the olecranon fossa also explains why an elbow effusion can distend the posterior recess and limit extension, and why arthroscopic debridement and posterolateral portal placement demand careful respect for the nearby ulnar nerve as it courses behind the medial epicondyle.
