Description
Shown in lateral profile, the temporomandibular joint (TMJ) region is framed by the squamous part of the temporal bone superiorly and the mandible inferiorly, with the mandibular condyle seated in the mandibular (glenoid) fossa and approaching the articular tubercle anteriorly. The fibrous capsule forms a loose sleeve around the articulation, attaching to the margins of the fossa and articular eminence above and to the neck of the mandibular condyle below. Deep to the capsule, the synovial membrane lines the joint and reflects around the articular disc, creating superior and inferior joint compartments that permit both hinge rotation and anterior translation. Posteriorly, the capsule blends toward the retrodiscal tissues, while the mandibular ramus continues inferiorly into the jaw. Capsular laxity is the mechanical reason the TMJ can translate, and it is also where pathology declares itself. Hypertranslation and anterior disc displacement often manifest as painful clicking and intermittent locking during mouth opening, while capsulitis and synovitis produce tenderness just anterior to the tragus and pain with resisted elevation. This view supports teaching the difference between rotation in the inferior compartment and translation in the superior compartment, a distinction that guides interpretation of TMJ MRI and informs intra-articular injection targets. Use this illustration in gross anatomy and head and neck courses when introducing synovial joint specializations, and in dental and maxillofacial modules covering temporomandibular disorders, occlusal splints, and arthrocentesis. It also fits operative and radiology publications discussing disc repositioning, arthroscopy portals, and the relationship of the capsule to adjacent temporal bone landmarks.
