Description
Running superficially over the posterolateral neck, the external jugular vein descends from the region just inferior to the auricle and crosses the surface of the sternocleidomastoid toward the root of the neck. Inferiorly it angles to pierce the deep cervical fascia in the supraclavicular fossa and terminate in the subclavian vein, lateral to the carotid sheath that contains the internal jugular vein and common carotid artery. Posterior cranial and upper cervical landmarks, including the occipital bone with its sutures and the cervical vertebrae with intervening intervertebral discs, provide fixed reference points behind the superficial venous course. Small accompanying cutaneous tributaries and adjacent neurovascular lines are shown in situ. Posterior views of the external jugular vein matter because the vessel is often accessed for rapid venous cannulation when peripheral access is difficult, and its superficial course makes it a reliable landmark when the neck is rotated. Vein distension along this pathway is also used at the bedside as a surrogate for jugular venous pressure, while focal prominence or asymmetry can reflect thrombosis, external compression at the thoracic inlet, or altered central venous return. The relationship to the clavicle and the subclavian vein explains why over-advancement or low puncture can increase the risk of pleural dome injury and pneumothorax. A short, practical pathway.
